Prom Date Permission Form
Permission to bring a date who does not attend SILO SCHOOL must be obtained by Wednesday, April 1st. NO DATES will be approved following the deadline. Your invited guest must accompany you through the door, be no younger than a freshman (2025-26 school year) in high school and must not be older than 19 years old at the time of Prom. You must be logged into your Silo Student Google Account to fill out the form. Every question must be completed in order to be considered. Click here to complete the form.
