Permission to bring a date who does not attend SILO SCHOOL must be obtained by Wednesday, March 26th.  NO DATES will be approved following the deadline.  Your invited guest must accompany you through the door,  be no younger than a freshman (2024-25 school year) in high school and must not be older than 19 years old at the time of Prom.  You must be logged into your Silo Student Google Account to fill out the form.  Every question must be completed in order to be considered.  If you are having trouble with the form see Mrs. Parr.  For any other questions, contact Mr. Atwood at j.atwood@siloisd.org. 

Information needed to complete the form:

• Dates parent’s name, address and phone number (in case of an emergency)
• Dates principal’s name, high school where they attend(ed) and the high school’s phone number
